Kaweah Health maintains access for Non-Kaweah Health Employees who require a remote connection to our Electronic Medical Record (EMR) systems for purposes of treatment, payment, or healthcare operations for our mutual patients. To request access, please complete the following application in its entirety. Incomplete applications will not be considered.
Please Note:
- All Non-Employee User Access Applications will be reviewed independently to determine necessity for access to Kaweah Health’s EMR systems.
- Kaweah Health reserves the right to approve, deny, or revoke access at any time.
