Exhibit space consists of a 6 foot table suitable for a table top display and comes with registration and WIC MACCS issued badges for two company representatives to staff the exhibit hall. Any additional representatives will be required to have an WIC MACCS issued badge which may be purchased by clicking here.

As the contact person ("Exhibitor"), I understand that all fees for the MACCS exhibit are being collected and must be delivered to the Maryland Chapter American College of Cardiology (“MDACC") prior to the beginning of the meeting. I am aware that space is assigned on a first come, first served basis. The MDACC will make all attempts at reasonable accommodation. This form may be completed in advance of payment if exhibitor has assurances that the exhibit fee is approved from company representatives/supervisors. Upon my signature of this Application, I agree: (1) the Exhibit Application becomes a binding contract between MDACC and Exhibitor; and (2) I have reviewed the Exhibitor Terms and Conditions (“Rules”) and will comply with the Rules and (3) I have reviewed the Meeting Code of Conduct (“Code”) and will comply with the Code and all other terms and policies hereafter adopted by MDACC (collectively, the “Rules” and "Code"). Payment

You may pay for your exhibit space by either the Maryland Chapter's secure Square site or by sending a check to the Pennsylvania Chapter Office. Your space will not be guaranteed until payment has been received.
