Practical Lab Rental Rules and Guidelines
To ensure a safe, efficient, and professional environment for all users, please adhere to the following rules when utilizing the Practical Lab at Inspire Health Medical Institute:

Booking and Payment
Advance Booking Required: All lab sessions must be scheduled in advance by calling our office. Walk-ins are not permitted.
Payment Policy: A rate of $25/hour (minimum 2 hours) applies and must be paid prior to using the lab.
No Refunds: Payment is non-refundable, including for unused or unfinished booked time. Please plan your session carefully.

Cancellations: 

A $35.00 cancellation fee will apply to any bookings canceled within 24 hours of the scheduled start time. This fee is necessary as the reserved time is blocked off and may not be rebooked by another student on such short notice.

Lab Usage
Unsupervised Practice: Lab sessions are unsupervised. You are responsible for your own practice activities during the session.
No Phlebotomy Practice on Others: Practicing phlebotomy on classmates or anyone else is strictly prohibited.
Supplies Provided: Limited basic supplies will be available. If you require specialty items, please inquire during booking.
Respect Booking Times: Arrive and finish your session on time to avoid scheduling conflicts with other users.

Group Practice
Cost Sharing Allowed: You may split the cost with classmates, but all participants must adhere to the rules. Only 2 people to a group are allowed. However, 2 groups can study together for a maximum of 4 people. If you have more than 4 people please call the office.
Authorized Participants Only: Only individuals who have booked and paid for the session are permitted in the lab.

Clean-Up and Maintenance
Clean As You Go: Leave the lab clean and organized. Dispose of waste properly and sanitize your workspace before leaving.
Report Issues: Inform staff immediately if you encounter any equipment malfunctions or supply shortages.

Safety and Conduct
Professional Conduct: Maintain a respectful and professional attitude at all times while in the facility.
No Food or Drinks: To maintain a sterile environment, food and beverages are not allowed in the lab.
Safety First: Follow all proper safety protocols and guidelines during your session.

Failure to comply with these rules may result in loss of lab privileges.

Hold Harmless Agreement Disclaimer

By utilizing the facilities at Inspire Health Medical Institute, all participants acknowledge and agree to the following:

Assumption of Risk: You understand that participating in activities or practicing procedures in the lab may involve inherent risks. By signing this agreement, you voluntarily assume all risks associated with the use of the facilities.

Hold Harmless Clause: You agree to release, discharge, and hold harmless Inspire Health Medical Institute, its staff, and affiliates from any and all liability for injuries, damages, or losses sustained while on the premises, whether caused by negligence or otherwise.

Personal Responsibility: You agree to exercise caution and follow all safety protocols while using the facility. Any injury or harm caused by your actions will be your sole responsibility.

Acknowledgment: By signing the Hold Harmless Agreement, you confirm that you have read, understand, and agree to the terms outlined above.

This agreement must be signed prior to using the facility. Failure to sign will result in denial of access to the premises.

Credit Card Authorization:

I authorize Inspire Health Medical Institute to charge the credit/debit card listed above for the specified amount.

I understand that this payment may be subject to non-refundable and cancellation fees as stated in the terms and conditions listed above.

This authorization remains in effect until the transaction is processed or canceled by me in writing.

By signing below, I confirm that I am the authorized cardholder and that I have read, understand, and agree to the terms and conditions outlined above.
