Program Details:
This waiver is for participation in the After-School Art Program held at Kaizen Artz Inc at 550 N. Morrison St. Appleton WI 54911.
Release of Liability:
In consideration of my child's participation in the After School Art Program, I, the undersigned parent or legal guardian, hereby release, waive, and discharge Kaizen Artz Inc, its instructors, volunteers, agents, and employees (collectively referred to as "Releases") from any and all claims or liabilities for injuries or damages, including, but not limited to, personal injury, property damage, or death, which may occur during the program or any related activities, whether occurring on or off the premises.
Assumption of Risks:
I understand that my child's participation in the After-School Art Program may involve inherent risks, including but not limited to, risks associated with art supplies, tools, Photography equipment, 3D printer supplies and studio activities. I acknowledge that I have made my child aware of these risks and that my child is voluntarily participating in the program.
Medical Emergency Authorization:
In case of a medical emergency, I give permission for program staff to seek medical treatment for my child and to contact emergency medical services if necessary. I understand that I will be responsible for any medical expenses incurred on behalf of my child.
Transportation Agreement
I am responsible for transporting my child to and from the art program. I am aware of the class time and agree to pick up my child after class. If there is a delay for any reason, I agree to communicate with staff.
Acknowledgment of Understanding:
I have read this Liability Waiver and Release of Claims and understand its contents. I am aware that by signing this document, I am waiving certain legal rights, including the right to sue the Releases.
