This Christian Coaching Agreement is made and entered into as of by and between Chassidy Mayo, Coach/Mentor and

Purpose The purpose of this Agreement is to outline the terms and conditions of the coaching relationship between the Coach and Client. The Coach will provide Christian coaching services to support the Client in their personal, spiritual, and professional growth.

The Coach agrees to provide the following services:

Consultations: Initial consultation to discuss goals, challenges, and ensure mutual compatibility. One-on-One Coaching Sessions: Personalized coaching focused on spiritual growth, emotional healing, and personal development. Support: Ongoing support via coaching sessions, email, or phone as needed.

Minors and Parent/Guardian Consent

If the Client is a minor (under 18 years old), a parent or legal guardian must provide consent for participation in the coaching program. The Parent/Guardian agrees to support the minor's participation and be responsible for ensuring the minor complies with the terms of this Agreement.

Term: The coaching relationship will commence on

Sessions: Coaching sessions will be held Weekly or Bi-weekly for 60 minutes.

Fees and Payment Fee Structure: The fee for each coaching session is $35. Payments are due upon booking at each session. Payment Method: Payments can be made via debit or credit cards, Zelle, or Paypal.

Cancellation Policy: Clients must provide at least a 24 hour notice to cancel or reschedule a session. Missed appointments without notice will be charged the full session fee.

Rescheduling: Rescheduling is subject to availability and ideally should be done within 1-2 week(s) from cancellation notice. Confidentiality All information shared during coaching sessions will be kept confidential and used solely for the purpose of providing coaching services, except as required by law or with the Client's consent.

Responsibilities Client's Responsibilities: The Client agrees to actively participate in sessions, complete any assigned tasks, and be open and honest during coaching sessions.

Coach's Responsibilities: The Coach agrees to provide support, guidance, and feedback based on Christian principles and ethical standards.

Either party may terminate this Agreement at any time with [Number of Days] days' written notice. Upon termination, the Client will be responsible for payment for any sessions rendered prior to termination.

The Coach is not a licensed therapist or counselor and does not provide therapeutic services. Coaching is not a substitute for professional medical or psychological treatment.
