Financial Policy
We share your concerns regarding the cost of health care. We believe that you, our patients, deserve the best possible care we can provide at a reasonable cost. With this in mind, we would like to share some information with you about our financial policy. We want you to feel comfortable with us regarding your financial and insurance matters and thereby prevent any misunderstandings. We hope you will consult with us if you have any questions regarding our service and/or fees.
Patients without Insurance:
Your account is required to be paid in full at the time of service. We offer a 5% courtesy to our patients without insurance when you pay your bill at the time of the appointment. We offer a 10% senior discount. Discounts will only be given with payment of cash or check.
Patients with Insurance:
Many people are under the impression that if they have insurance, it is the insurance company that owes the doctor. This is not the case. The insurance contract is between the patient and the insurance company; therefore the patient is responsible for the bill regardless of insurance coverage. As a courtesy, we are happy to submit claims for you, however, it is the responsibility of the patient (or insured) to provide our office with the correct insurance company name, address, telephone number, appropriate identification numbers, and the patient’s and insured birth dates. Please be aware that some insurance companies will pay a claim based on their “usual and customary”, not our actual fees. We do our best to calculate what your estimated portion will be for each visit and that portion is due at the time of service. All insurance companies have a maximum yearly amount paid on a calendar or benefit year. Check your policy to see if you have a deductible and if your insurance pays at a percentage or by a fixed fee schedule.
Broken Appointments:
A minimum fee of $50.00 may be charged for ALL broken appointments due to operating expenses. To avoid this fee we request a 48 business hours’ notice so your appointment can be given to another patient.
Payment Options:
We accept Visa, Mastercard, Amex, Discover, cash, check and Care Credit. We don’t offer extended payment plans. All account balances are due within 90 days of treatment.
Delinquent Accounts:
Delinquent accounts may be referred to our collection agency after 120 days. You are responsible to pay any collection costs and reasonable attorney fees incurred to effect collection on the account. Accounts assigned to collections will be charged a $75.00 fee.
Returned Checks:
A $25.00 service charge may be assessed for all returned checks.
