I understand that there are inherent risks in participating in strenuous exercise/competitions. If I sustain or claim to have sustained an injury whilst participating in exercise within these facilities, I acknowledge that the gym is not responsible, and with the exception of any gross negligence or malicious acts, the trainers/owners cannot be held liable for the effects from the physical exercise and the adjustments that my own body may experience throughout a training programme designed to help me achieve my goals or from a competition.

I understand that the gym has full Public Liability Insurance through an approved Insurance company and the Insurance is for the purpose of cover for any injuries sustained whilst training/competing, as well as a coaching/personal training appointment/session

I certify that my answers to the questions outlined in my Par Q Questionnaire (to be filled out during weigh in) or any other form of Questionnaire/ Interviewing that the gym or my trainer conducts are true and to the best of my knowledge. I understand and agree that it is my responsibility to inform the gym and/or my trainer of any conditions or changes to my health, now and ongoing, which might affect my ability to exercise/compete safely.

Entry fee's are as follows:

1 event: £20.00

2 events: £30.00

3 events: £40.00

Please ensure you send the correct amount for the events you have selected.

You are not officially entered until your entry fee has been received. Refunds can't be issued under any circumstances should you later be unable to compete, and spaces for the competition can't be reserved. The competition will be full when we reach 20 competitors per event.

If you need to change anything on your form, please email us, do not fill out another form. Once you have paid your entry fee and submitted a form, you will receive email confirmation from Jotform. You are then entered.
