People who fill out this form to register in advance will be given priority for appointments. SERVICE DELIVERY MODELS

We provide FREE Income Tax Preparation Services through our statewide partners on the IRS-sponsored Volunteer Income Tax Assistance (VITA) Program. SimplifyCT and our partners have a variety of different service models. Please read the information below prior to making a selection:

In-Person Assistance:

Schedule your FREE income tax assistance appointment at one of our various locations. If this option is selected, you will receive a message in mid-January to schedule your appointment through a local partner.

Virtual VITA:

Submit your documents electronically through our secure Virtual VITA Program sponsored by Code for America and GetYourRefund! A link for secure document upload will be sent out once the platform is open in mid-January and your return will be expedited.

"Do It Yourself" through IRS Direct File and TaxSlayer FSA:

We will send you a specialized link to upload your documents and complete your return for FREE on your own through IRS Direct File or our software, TaxSlayer. Certified Volunteers will be available to assist you along the way with any questions you may have.

Photo identification or valid driver’s license

Social Security cards or Individual Taxpayer ID Number(s) for all household members

Birth dates for you, your spouse, and your dependents

Wage and earning statement(s) forms W-2, W-2G, 1099-R, from all employers

Interest and dividend statements from all banks (Form 1099)

Forms 1095 A, B or C, Affordable Care Act Statements

Copy of last year’s federal and state returns, if available

Proof of bank account routing numbers and account numbers for direct deposit (blank check or a letter from your bank)

Other relevant information:

Receipts from child care providers, with provider’s identifying number

Education expenses and student loan information

List of all business income and expenses, if applicable

Married taxpayers filing a joint return: both spouses must be present to sign required forms
