I waive and release any and all rights, claims, or causes of action for damages or injuries to myself of any kind whatsoever arising out of or in connection with my participation in the 2026 Life in the Spirit Seminar Retreat facilitated by Family of Faith (FOF) Covenant Community. I hereby waive, release, and forever discharge FOF and its members of any liability from any such claim.
In the case of intentional property damage caused by myself, I take full responsibility for the cost of such damage. If I become ill, destructive, or display any abnormal behavior, it is my obligation to report this to a FOF retreat staff and leave retreat premises upon requested notice.
In reading and signing this e-document, I agree to these terms stated above and that FOF reserves the right to revoke my registration with no questions asked. I additionally agree that FOF is not obligated to refund me if I fail to request a refund before the refund deadline (01/31/2026), which will be refunded via check.
