Parental Consent for Photography and Video Recording

Dear Parent/Guardian,

At Psi Alpha Chapter of the Omega Psi Phi Fraternity, Inc. we are dedicated to celebrating and showcasing the achievements and activities of our students. To share these moments with our community and promote our programs, we may capture photographs and video recordings of students during events, activities, and programs.

We request your permission to use photographs or videos featuring your child for the following purposes:

• Marketing and Promotional Materials: Includes brochures, flyers, posters, and other printed materials.
• 
  Online Platforms: Includes our website, social media channels, and other digital platforms.
• 
  Media Coverage: Includes local newspapers, magazines, and other press-related activities.

Please Note:

Your child’s name will not be published without additional explicit consent.
Photos and videos will be used responsibly, in alignment with our mission to highlight student achievements and activities.

If you do not grant permission, we will ensure your child is not included in photographs or videos used for these purposes.
