Please fill out the form below to be scheduled with Seth! Each individual person getting tattooed must fill out their own form individually! Multiple clients per request will not be accepted.
By selecting this, I agree that I am at least 18 years of age and that I am filling out this questionnaire on behalf of myself. I agree that I am not filling out this questionnaire for another person. I understand that if I fill out this questionnaire for more than one person or more than one tattoo description that the request will not be accepted. I also understand that due to shop capacity, I am not allowed to bring a guest along to my tattoo appointment.
After you have sent in your request, you will get a reply back that we have received it along with instructions that will ask for reference photos. (please allow us 1-4 days to get your confirmation to you, a real person will be sending each response!). Everyone who sends in a request will receive a reply email back! If your project is chosen, we will email over an appointment date/time to you after we have gone through requests. You will have 3 days from the date of the email to confirm the appointment time. When you have emailed back to confirm, we will then send you a booking fee invoice to lock in your appointment. Your invoice will have a due date to be paid by.

Include the main elements of your tattoo as well as smaller details (Ex. the specific type of flower(s) you want, the EXACT wording or any dates/times you want). Please refrain from giving extra details that don't pertain to the actual tattoo design.

Where on your body would you like your tattoo placed? Please be specific when describing the placement include which side of the body your tattoo will be on! (Ex.: upper, outer right arm, inner left bicep, lower left leg, etc.)

Is this tattoo a cover up or continuing project? If either are a yes, you will be asked to send in photos of the existing tattoo.

In the next box, we will ask about your appointment availability. By selecting "Yes" below, you agree that your availability is accurate for the months that we are booking. When selecting your availability, please choose TWO "All Day" availability options. We cannot guarantee Saturday availability. Before selecting your availability, please be aware that we book your appointment based off of the days you choose and the work schedule that you send. We book Seth three months out to ensure that anyone who works Monday-Friday shifts can schedule the time off needed for an appointment. Please know we may not have an alternate date to offer you as we have other clients waiting for an appointment

What days & times during Seth's shop hours are you able to come in for an appointment? When selecting your times, please be aware that you must be available during the entire time frame from 11am-6pm. Saturday appointments are the first to fill and not always guaranteed available. If you choose Saturday as your first choice, please choose two other available days as well! (There must be at least two available times selected)

Please note any vacation dates and/or times you may be out of town. This will help avoid any scheduling conflicts! If you are scheduled for trip that will involve, water/sand/sun please let us know ahead of time!

Please note emails are answered Tuesdays-Saturdays and our turn around time can be anywhere from 3-7 business days depending on the email flow for our studio. If you are awaiting a reply to your questionnaire and haven't seen anything in your email, please check your spam and junk folders!
