Client Agreement for Credit Repair Services By signing this form, I acknowledge and agree to the following: 1. Disputing items on my credit report does not eliminate my financial responsibility to pay those items. 2. I am engaging Sweet Savings Financial to assist in removing negative items from my credit report. If no items are removed within 90 days, I will be eligible for a full refund. 3. While my credit report is being disputed by Sweet Savings Financial, I will refrain from applying for any new credit or loans. Additionally, I will ensure timely payments on my existing accounts to support the improvement of my credit score. I understand that Sweet Savings Financial's services focus solely on the removal of negative items. 4. After submitting this form, I will write my full name only in the invoice space provided on the next page and complete payment through this provided invoice. 5. The service fee of $250 covers three months of credit dispute services. During this period, I agree to maintain my subscription with IdentityQ, which will bill me separately at $29.99 per month. By signing this form, I confirm that I have read, understood, and agreed to these terms.

After submitting this form and payment, you will receive an email with onboarding instructions so the process can be completed.

Steps to Prepare:

1. Sign up for credit monitoring at identityQ https://www.identityiq.com/and purchase the $29 plan. You MUST select the $29 plan.

2. Create a Credit Karma account at creditkarma.com. If you already have an account, we’ll use your existing login information.

Once you complete these steps, be prepared to reply to the onboarding email you will receive shortly after submitting this form with the login credentials from your IdentityQ and Credit Karma account.

Credit monitoring service is mandatory to proceed with this process.
