Is aware of the risk they are exposed to during the activity and that it can lead to injury or death,  b. Is aware and accepts the risks involved and accepts they are responsible for their own actions and/or involvement,  c. Is fit and capable of participating,  d. In respect of participants under 16 years of age all registration forms are countersigned by a parent or guardian, g) All equipment must be operated in accordance with manufacturers’ recommendations and a record must be kept of the appropriate checks completed on any equipment, h) That Personal Protective Equipment is supplied to all Employee as required and the use or wearing of Personal Protective Equipment by all Employees where required is rigorously enforced.

A material circumstance are circumstances which may influence an insurer's decision to cover a risk and/or the terms that are applied.
