PRO-TRAINING INJURY AND GENERAL LIABILITY RELEASE FORM

By signing your name below as a participant and/or parent/legal guardian of a participant in Pro-Training, you acknowledge that participation in the program exposes you to the possibility of personal injury. You, being fully aware that participation in the program exposes you to a potential risk of personal injury, hereby release Pro-Training and its volunteers, directors, employees, agents, licensees, consultants, independent contractors, and affiliates from all liability for property damage, personal injury, or other claims arising out of or in connection with your participation in the program, including claims known and unknown, foreseen and unforeseen, future or contingent.

You acknowledge that you have read and fully understand the Injury Liability Release and General Liability Release.

PRO-TRAINING PHOTO RELEASE FORM

Pro-Training has my permission to use my photograph publicly to promote its program. I understand that the images may be used in the printed brochure, website, and social media. I also understand that I will not be paid royalties, fees, or any other compensation for such use.
