  • Events should be planned 3 months in advance to ensure the success of your event. Please be able to explain all or most of the following information 3 months prior to the event date. If any information is unknown at this time, you may indicate "unknown" but note that staff will contact you soon for further clarification. We understand that changes may be made to this plan. You may be contacted by various staff members to clarify different aspects of this event upon receipt of your planning form. Please designate and list the primary contact(s) for the event so that we reach out to the correct person. 

    Note: This form is a starting point and there may be other forms required depending on the complexity of your event. Our staff will contact you if further information or forms are needed.

  • Outside Organization Acknowledgements & Agreements

    The Organization requesting the building use will be referred to hereafter as the USER. The representative requesting the building use will be responsible for the communication between the USER and First United Methodist Church.
  • BUILDING USE FEES

    Building Use Fees will be calculated according to the Building Use Policy Fee structure. Submitting this form is a tentative booking and a member of staff will reach out to you regarding facilities pricing. All fees must be paid in full before the event is placed on the First United Methodist Church calendar. 

  • PROPERTY DAMAGE INSURANCE

    USER also at its sole cost and expense shall maintain during the use of the facility property damage limits covering the facility to be used of not less than $1,000,000.

    It is the intention of both USER and First United Methodist Church that both the public liability and property damage insurance shall insure performance by USER of the express indemnity provision contained below. However, the limits of such insurance shall not limit the liability of USER hereunder.

    First United Methodist Church shall be named as an additional insured on the insurance policy purchased by USER, which is the subject of this agreement.

    USER agrees to provide First United Methodist Church with a copy of the certificate of insurance evidencing that it has complied with the insurance requirement of this agreement. 

  • EXPRESS INDEMNITY

    USER agrees to save, indemnify, and keep harmless First United Methodist Church against any and all liability, claims, judgments, or demands, including demands arising from injuries or death of persons (USER'S employees included) and damage to property, arising directly or indirectly out of obligations herein undertaken or out of the operations conducted by USER, save and except claims or litigation arising through the sole negligence or sole willful misconduct of First United Methodist Church. It is the intention of the parties that the indemnity provided for by this agreement provides for indemnity to the fullest extent provided for by law.

  • PARTICIPANTS OF MINOR AGE and/or VULNERABLE ADULTS

    Prior to utilizing First United Methodist Church facilities for activities that include participants of minor age and/or vulnerable adults, all adult leaders and assistants must attend Ministry Safe training and will be required to sign First United Methodist Church's Ministry Safe Policy. USERS will promptly report abusive or inappropriate behavior to the appropriate authorities and Senior Pastor or Business Administrator. USERS using First United Methodist Church's facilities must be re-trained upon Ministry Safe expiration. 

  • MATERIAL GUIDELINES

    First United Methodist Church reserves the right to request a copy of any materials used or handed out within their property. The contents of the materials must be approved by the Business Administrator or Getsemani Program Director. This approval is required 2 weeks in advance of the event date.

  • The undersigned has been given authority to act for and be responsible for the USER making this application. USER will see that the FACILITY is not misused or abused, that there is proper adult supervision at all times, that the FACILITY is used in conformity with all policies and regulations of First United Methodist Church, and that all other terms of this Building Use Agreement are adhered to and followed.

    The undersigned understands and agrees that this Building Use Agreement does not establish an employer-employee relationship between USER and First United Methodist Church, that the event is neither a conducted event nor a sponsored event of First United Methodist Church. In addition, it is understood that First United Methodist Church will not exercise any physical or other control over the operation of the event other than those already specified in this Building Use Agreement. In addition, USER understands that First United Methodist Church is not providing any supervision by this agreement.

  • Important Information for Upcoming Events:

  • If your event is during regularly scheduled church hours in the main building (Monday-Thursday, 9:00 AM to 3:00 PM), the doors will be unlocked as they usually are.

    If your event is in the main building after hours, only the glass doors (by the playground) will be unlocked 15 minutes prior to your meeting’s start time and will lock 15 minutes after your meeting’s start time. No other doors will be unlocked.

    If your event is in the MCMC building, the doors will be unlocked 15 minutes prior to your meeting’s start time and will lock 15 minutes after your meeting’s start time. No other doors will be unlocked.

     Please relay these changes to the members of your group.

    If you need to cancel your event after submitting this form (especially if you have child care arranged through the church) please email cancellations@fumcgt.org as soon as possible.
