Lobos Den Group Exercise Waiver and Release Form

Please read the following carefully before signing.

I, voluntarily enroll in the health and fitness programs offered by Lobos Den, which may include the use of Herbalife Nutrition products and services. I acknowledge that these programs may involve strenuous physical activity, including but not limited to muscle strength and endurance training, cardiovascular conditioning, and other fitness-related activities.

I affirm that:

• I am in good physical condition and free of any medical conditions or disabilities that could prevent or limit my participation.

• I understand that participating in physical activities and consuming nutrition products carries inherent risks, including physical injury, adverse health reactions, property damage, or even death.

ASSUMPTION OF RISK AND RELEASE OF LIABILITY

I hereby:

• Voluntarily assume full responsibility for any risks, injuries, or losses arising from my participation in Lobos Den programs or the use of Herbalife Nutrition products.

• Release, waive, and discharge Lobos Den, Herbalife Nutrition, their respective owners, officers, agents, employees, independent distributors, and volunteers from any and all claims, liabilities, or causes of action of any kind, including those arising from negligence.

• Agree not to hold Lobos Den or Herbalife Nutrition liable for any damage or loss of personal property during participation in activities or while on the premises.

ADDITIONAL AGREEMENT

• I acknowledge that Herbalife Nutrition products are not intended to diagnose, treat, cure, or prevent any disease.

• I agree to consult a physician before starting any fitness program or using any new nutrition products.

• I understand that this waiver extends to my family, heirs, assigns, and personal representatives.
