I am of legal age, 18 years or older.
I consent to contributing a minimum donation of £2.75 for each hen.
I commit to offering a permanent home for the hens or ducks for the entirety of their natural lives. If I am no longer able to care for the hens or you no longer want them, I will get in touch with Fresh Start For Hens.
I will ensure that professional veterinary care is sought whenever necessary.
I will provide or have provided photos of my coop and free range area, and are accurate at the time of sending, within 3 days of submitting this form.
I understand that it is necessary to make my donation promptly, within 3 days of placing my reservation.Failure to do so will result in the cancellation of my reservation and the forfeiture of receiving the hens or ducks.
I acknowledge that Fresh Start For Hens reserves the right to decline any reservation, and I will be notified of the reasons for rejection via email.
