I have read, understand, and agree to the Terms and Conditions of my/and or my child's enrollment at Engage Art Studio listed near the bottom of the "Kid's Summer Camp" tab. I have also read, understand, and agree to Engage Art Studio's Summer Camp Payment and Refund Policy. I understand that if I do not want pictures of my child to be uploaded to Engage Art Studio's social media, I must notify my child's teacher prior to camp.

I understand that Engage will send a confirmation email within 72 hours of me submitting this form. I understand that after my confirmation email is sent, I will have one week to pay the deposit amount listed in said confirmation email. I understand that if my deposit is not paid within one week (7 days) after my confirmation email is sent, I forfeit my space at camp and must either re-register or call/email to confirm that a space is still available.
