If your secondary identification is NOT a photographic id, then we also need authentication by an identity referee. The identity referee shall meet the following criteria:
· They must have known you for at least 12 months
· They must be at least 16 years of age
· They must not be related to you and not part of your extended family
· They must not be your spouse or partner
· They must not live at the same address as you.
AND either:
• A photograph of the person authenticated by the identity referee. The identity referee needs to write on the back of the photograph “Certified true likeness of [full name of person]” (or words to that effect), and sign and date the certification.
OR
• A verification statement signed and dated by the identity referee verifying that the primary identity document produced by the person relates to that person.
