Submissions are reviewed every week on Mondays.

There is no appointment without deposit, deposits are non-refundable, filling out this form does not guarantee and appointment.

NO MINORS will be tattooed

NO ONE UNDER THE INFLUENCE of drugs or alcohol will be admitted

CASH PREFERRED (Deposits can be made through venmo)
