HALO Transition Student Interest Questionnaire

All HALO Transition application materials must be received by July 6, 2026.
The Student Interest Questionnaire helps us understand what you enjoy, what motivates you, and how you believe the HALO Transition Year Program can best support your goals for school, work, and independence. Please complete the two‑page questionnaire on your own as much as possible so we can learn from your own voice and perspective. If someone supports you while completing it, just make sure to note that in the space provided at the bottom of page two.
