Use of Voice, Image, Likeness and Performance Waiver Form

The undersigned hereby irrevocable grants Summit Music Festival (“SMF”) permission to use my own and/ or my child’s voice, image, likeness, or any musical performance at SMF which may include myself and/or my child (collectively, “performance materials”), in any broadcast, photograph, video, internet site, audio recording, and in any and all of SMF’s current and future publications (collectively “promotional materials”), without pay- ment or any other consideration.

I understand that these performance materials will become the property of SMF and will not be returned. I hereby irrevocably authorize SMF to edit, alter, copy, exhibit, publish, adapt, perform, reproduce, modify, make derivative works, distribute or otherwise use any of the performance materials, for the pur- poses of publicizing or promoting SMF’s services or programs or for any other lawful purpose. Such use includes, without limitation and in the sole discretion of SMF, the display, distribution, publication, transmission, or otherwise use of the performance materials, in any format now existing or developed in the future. In addition, I waive the right to inspect or approve the finished product of any promotional materials, including written or electronic copy, wherein the performance materials, or any part thereof, appear. Additionally, I waive the right to royalties or any other compensation arising or related to any use of the performance materials, now or in the future. SMF may exercise any of these rights itself or through any successors, transferees, licensees, distributors, assigns or other parties, commercial or nonprofit.

I hereby hold harmless and release and forever discharge SMF, its successors, transferees, licensees, distributors and assigns from all claims, demands, and causes of action which I, my heirs, representatives, executors, admin- istrators, or any other persons acting on my behalf, my child’s behalf, or on behalf of my child’s estate have or may have by reason of this authorization.

All participants & boarding guests are required to sign the following:

• I am expected to be cordial to my peers, respectful of all teachers and abide by the Rules and Policies of the Summit Music Festival.

• I am to respect University of Mount Saint Vincent property. This includes all rehearsal spaces, dorm rooms, and lounge areas. I will keep the rooms orderly and clean.

• I will show respect for others by taking all commitments seriously – lesson appointments, rehearsal schedules, classes, concerts, and performances. I am responsible for knowing my own schedule and to be on time.

• I will comply with all security and safety protocols of University of Mount Saint Vincent. I will have the SMF/ University of Mount Saint Vincent Identification Card in my possession AT ALL TIMES while on campus.

• I will abide by the Alcohol and Drug Policy of University of Mount Saint Vincent as outlined in the Policies.

• I will abide by the Non-Smoking Policy of University of Mount Saint Vincent.

• I understand and will follow the Summit Music Festival curfew policy.

• I understand that failure to comply with any of the Rules and Policies of the Summit Music Festival and University of Mount Saint Vincent will result in disciplinary action which may result in dismissal from the Summit Music Festival program (with no refund).

AUDITION VIDEO
An audition video must be sent with your application. At least one video is required.

Please review the instructions below on labeling your video and the size restrictions. Audition video is required for all applicants for placement in the programs (orchestra, chamber music, student galas, etc.) Video maximum upload size is 1GB in mp4 format.

Prepare two contrasting pieces best representing your level. Please put the video(s) on YouTube and send us the link(s).

PAYMENT INFORMATION & AGREEMENT
Application Fee

A $180 non-refundable application fee to be paid upon submission of your application.

PAYMENT TERMS

Upon Acceptance to the program, 50% of the tuition is due to hold your place in the program. Balance of tuition and room & board fees must be received no later than May 15, 2025.

CANCELLATION & REFUND POLICY

Cancellations made prior to April 30, 2025 are entitled to a full refund.

Cancellations made April 30, 2025 trough May 15, 2025 will result in a $500 cancellation fee.

Cancellations after May 15, 2025 are NOT entitled to a refund.

Payments must be in US dollars, payable to the Summit Music Festival, Inc. and drawn on a United States bank. Money orders from a US bank are also accepted. We also accept Visa, MasterCard, American Express and Discover.

Invoice will be generated to the email on the application with payment options.
