1. A seperate form must be submitted for each team.
2. Coaches with multiple teams must ensure that the same head coach is listed for each team in GotSport for the H-TOWN SOCCER LEAGUE league.
3. The H-TOWN SOCCER LEAGUE may not be able to accomodate all scheduling requests.
4. All pre-season scheduling requests must be received by the deadline posted on the league website.
