Alternate Pet Pick Up and Drop Off Consent Form

By completing and signing this Alternate Pet Pick-Up/Drop-Off Consent Form, I, the pet owner, acknowledge and agree to the following terms:

1. Authorization of Alternate Individual. I hereby authorize the individual(s) listed on this form to pick up and/or drop off my pet(s) to and from Fire Hydrant Pet Sitting & Doggy Daycare. This authorization remains in effect until written notice of revocation is provided by me to Fire Hydrant Pet Sitting & Doggy Daycare.
2. Responsibility for the Pet. I understand and agree that once my pet(s) are released to the authorized individual(s), Fire Hydrant Pet Sitting & Doggy Daycare, its employees, agents, and representatives are no longer responsible or liable for the care, safety, or well-being of my pet(s).
3. Verification of Authorized Individual. Fire Hydrant Pet Sitting & Doggy Daycare reserves the right to verify the identity of the authorized individual(s) before releasing my pet(s). This may include requesting photo identification. I understand that failure to provide proper identification may result in a delay or refusal to release the pet(s).
4. Release of Liability. I release and hold harmless Fire Hydrant Pet Sitting & Doggy Daycare, its owners, employees, agents, and representatives from any and all liability, claims, damages, or actions that may arise once my pet(s) are released to the authorized individual(s).
5. Notification of Changes. I agree to notify Fire Hydrant Pet Sitting & Doggy Daycare immediately of any changes to the authorized individual(s) listed on this form. Until such notification is received and acknowledged, the existing authorization remains valid.
6. Acknowledgment of Understanding. I acknowledge that I have read, understood, and agree to the terms outlined in this agreement. I understand that this consent is a binding agreement and serves to protect Fire Hydrant Pet Sitting & Doggy Daycare from liability once my pet(s) are in the care of the authorized individual(s).
