  • Payment Option Details for Florida Residents

    1. Self-Pay: Pay with a credit or debit card.


    Due to the complexities of coverage from different insurance companies, we provide the following payment options to our Florida patients:

    2. Self-Pay with Insurance Submission: (Select Plans) - Payment is processed before the visit. For the following insurances; Cigna, Oscar, Aetna, Emblem, Freedom/Optimum, Humana, Medicare, Multiplan, Prime Health, VACCN, or RR Medicare, we will submit a claim on your behalf and reimburse you, any refunds, minus any applicable co-pays, within 30 days of receiving the Explanation of Benefits (EOB) from the insurance company.

    3. Self-Pay with Insurance Claim: For insurance plans that we do not accept, you pay for the visit. We will then provide you with an HCFA 1500/CMS 1500 claim form to submit to your insurance company.

  • Our system is fully HIPAA compliant. To enhance privacy, consultation orders are processed as a discreet "Consultation" service. On your statement, it will appear as "MCONSULT," ensuring your health information stays secure and confidential.
