Informed Consent and Acknowledgement
By signing your name below as a participant, and/or parent/legal guardian of a participant in Prp-training, you acknowledge that participation in the program exposes you to the possibility of personal injury. You, being fully aware that participation in the program exposes you to a possible risk of personal injury, hereby release Pro-training and its volunteers, directors, employees, agents, licensees, consultants, independent contractors, and affiliates from any and all liability from property damage, personal injuries, or other claims arising from or in connection with your participation in the program including claims that are known and unknown, foreseen and unforeseen, future or contingent.
You acknowledge that you have read and fully understand the Injury Waiver and General.
PRO-TRAINING PHOTO RELEASE FORM
PRO-TRAINING has my permission to use my photograph publically to promote their program. I understand that the images may be used in the print brochure, website, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
