Teams planning to play at the facilities listed below must complete this form. For all other facilities, register your team in the Private Facility flight on TennisLink.
- Alexander Street Park (no lights, however 6:30pm time slot will be scheduled)

I understand that by submitting this request form, I am committing to have the minimum number of players registered on the the team within 48 hours of receiving a team number from the Local League Coordinator. If the player minimum is not met by the specified date and time, the team will be removed from TennisLink and be moved to the bottom of the request list, if re-entry is requested.
