Membership agreement, release and assumption of risk
1. I acknowledge that the course for which I'm applying to may have unanticipated risks that could result in physical or emotional injury, permanent disability, paralysis, death or damage to myself, to property or to third parties. I understand that such risks cannot be mitigated without sacrificing the overall quality of said
2. I agree that the risk of injury is assumed by me, regardless of whether the injury or damages were caused by negligence of the club, its employees or its agents.
3. I hereby voluntarily release, forever discharge and agree to indemnify and hold harmless Maxwell Brazilian Jiu Jitsu LLC from any claims, demands or causes of action, which are in any way connected with my participation in this activity.
4. By enrolling with Maxwell Brazilian Jiu Jitsu LLC, I, my personal representative, assigns, heirs, successors in interest, and next of kin hereby release, waive, discharge and covenant not to sue Maxwell Brazilian Jiu Jitsu LLC, its owners, perhaps principals and instructors all for the kipurposes herein referred to from all liability to me, my personal representatives, assigns, heirs, successors in interest and next of kin for all loss or damage whatsoever, and any claim or damage therefore, on account of injury to my person or property or resulting in my death, in connection with my association with or entry in the courses I am participating in. This waiver and release of liability applies to all activities, without limitation, regardless of whether participation requires the participant to leave the premises of Maxwell Brazilian Jiu Jitsu LLC.
5. I agree to abide by the rules and regulations of Maxwell Brazilian Jiu Jitsu LLC, and at all times while on the premises, or at any sanctioned contest or exhibition of Brazilian Jiu Jitsu. I understand that Maxwell Brazilian Jiu Jitsu assumes no responsibility for lost or damaged personal property.
6.I understand that videos or pictures taken take of me or in connection with this program may be used by Maxwell Brazilian Jiu Jitsu LLC for publicity or promotion without compensation.
7. Maxwell Brazilian Jiu Jitsu LLC reserves the right to terminate any student from participation in the course for fighting, assault, Orr other such acts of violence that are in violation of the law.
By signing this document, I acknowledge that I have read this agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by the law. I have had sufficient opportunity to read this entire document. I have read and understood it and agree to be bound by its terms.
