Dos’ Tattoo Consultation Form
These are the questions I ask all of my clients in person. Please be as descriptive as possible and I will contact you to discuss the details of your request. Please be aware there will be a $35 deposit for the first hour and an additional $25 for every subsequent hour booked for your tattoo. All but a $10 booking fee of your deposit will be deducted from the total tattoo cost. YOUR APPOINTMENT IS NOT BOOKED UNTIL THE STUDIO IS CONTACTED AND THE DEPOSIT IS PAID. ALL DEPOSITS ARE NON-REFUNDABLE. A 24 HR NOTICE CALLED IN TO THE STUDIO WILL ALLOW FOR ONE RESCHEDULE WITH YOUR PAID DEPOSIT. No services offered for those under 18 years of age.
