Camp Waiver and Acknowledgement

By registering my child for this St. Albans Camp program, I attest that my child is in good physical condition. My child has no physical impairment or condition that will prevent his/her participation in camp activities. I give my permission for the St. Albans staff to seek professional medical care if my child requires it.

I agree to abide by all program rules and policies and teach my child to do the same. I will assume all risk associated with my child's participation in this program. I represent and affirm that my child is in proper physical condition to participate and will not hold St. Albans, responsible for his/her personal injury.

Refund policy: A one-week notice is required to cancel any session in order to receive a full refund. Camp Staffing is planned ahead of time and is based on the number of children enrolled. Day to day absences from camp will not be refunded. There will be no refunds after the first day of camp.

Photo/Video Release: Additionally, I grant St. Albans permission to use my child’s name, image, voice and identity in any program for promotion of St. Albans including written materials, website materials on any and all St Albans publication across the entire worldwide web including but not limited to saintalbans.com and its sub domains, Facebook, Twitter, Instagram and any other unnamed form of social media, video, audio or other forms without prior consent or compensation of any kind.
Swim Waiver and Authorization

I, the parent/guardian of the registered athletes, agree that the participants will abide by the rules of St. Albans Swim Club. Recognizing the possibility of physical injury or death associated with swimming and diving activities. I hereby release, discharge, and/or otherwise indemnify St. Albans Swim and Tennis Club, Phillip Hahn, Attack the Net LLC, its affiliate organizations and sponsors, their employees and associated personnel, including the owners of the facilities, board members, volunteers, agents, contractors, coaches and other volunteer persons used for the programs, against any claim by or on behalf of the registered athletes as a result of the athletes participation in the programs, and/or being transported to or from the same, which transportation I hereby authorize.
