  • Why Choose Our After-School Program?

    Have you ever dreamed of an after-school program that offers daily activities such as tutoring, guitar, piano, art, baking, cooking, individual and group therapy, caregiver and sibling therapy, life skills, and skill-building? Jacob's Mission Community Center, a progam of ASA Now, is thrilled to partner with Living and Balance Wellness Center to offer an exceptional after-school therapy program, designed to meet the unique needs of your household.

    We understand the importance of creating a safe, structured, and inclusive space where every child feels valued and supported. This program is thoughtfully designed to foster emotional resilience, build essential life skills, and help your child thrive in school, at home, and beyond.

    Empowering Your Child for Success:

    We understand that every caregiver wants the best for their child and loved ones in their household. Our after-school program is designed to provide the tools, support, and community needed to help your child succeed. Join us in creating a brighter future for your household.

    Living and Balance Personalized Coaching & Care:

    Our Monday and Thursday small group and one-on-one sessions with behavior coaches provide personalized support, helping your child, their siblings, and caregivers reach their full potential in a safe, nurturing, and understanding environment. Group sessions will be capped to five students per clinician. Dinner is provided for every student in attendance.

    Jacob's Mission Community Center:

    Our center is open Tuesday and Wednesday, from 2:00 PM to 7:00 PM. Snacks and dinner is provided for every student in attendance. Each K-12 school-aged child is welcome to receive services anytime between 2:00 PM and 7:00 PM, Monday and Thursday is behavioral health services through Living and Balance and Tuesday and Wednesday is community based services through Jacob's Mission Community Center.

    We are honored to serve children in grades K-12 and their caregivers at our community center, located in East Mesa at 7830 E. University Drive, Mesa, AZ 85207, between Sossaman and the 202 Red Mountain Freeway.

    Program Details:

    • Program runs Monday through Thursday 2 - 7 p.m.
    • Each class will be 45 minutes long.
    • Please let us know which services you would like your child, caregivers, and any siblings to attend. This interest form helps us understand your availability and the services you are interested in. Once the classes and schedules are finalized, we will send you a follow-up email with more details.


    Important Dates:

    • The program begins on Monday, January 27, 2025
    • Caregiver Workshops start in February 2025.
    • No classes the week of March 10th, 2025 for JMCC students. Living and Balance will have a Day Program. 

    What does the program include?

    • Fun and engaging activities

    • Life skills workshops

    • Emotional and therapeutic support

    • Snacks for participants (we are working on getting Dinner)

    • Caregiver workshops starting in February 2025

    Cost to Participate:

    • Therapeutic services provided by Living and Balance will be billed through the child's insurance. If child does not have insurance, there may be eligibility for financial assistance through other programs. If you have selected for your child to receive therapeutic services, your information will be shared with them and they will reach out to do an intake. 
    • JMCC Tutoring - services are covered through scholarships and there is no cost to the family.
    • JMCC Classes - services are covered through scholarships and there is no cost to the family.

    What is the attendance requirement?

    Students must attend at least 2 days per week to remain enrolled in the Living and Balance therapeutic program Services. Once a week are available for students participating in tutoring or activities only.

    How is transportation handled?

    Transportation may be requested and coordinated for therapeutic services offered by Living and Balance based on your insurance provider's rules and regulations. Living and Balance will provide additional information during intake. Please note that for students attending tutoring or classes through JMCC only, transportation will not be provided.

    Enrollment Information:

    Please note that this complete enrollment form will only need to be filled out once for each child in your household. Below is the required information for each child:

    • Photo or File of Notice To Provider (NTP), Tribal or DCS Guardianship Paperwork for Foster Children in your Care or Health Insurance Card for Biological or Adopted Children to verify age and identity.

    If you select for your child or household to attend therapeutic services, your submission information will be shared with Living and Balance. 


    Volunteer Information:
    Please note that any volunteers working with children and youth at our community center must pass a background check and attend an orientation and trauma training before volunteering with your child.

    skills like deep breathing and grounding for
    emotional regulation.

    into lyrics while learning musical skills.

    collaborate while building practical life skills.

  • Description of Service Opportunities

    We offer many different after-school activities at Jacob's Mission Community Center. Each K-12 school aged child and their caregivers in the household is welcome to attend Monday through Thursday between 2 pm and 7 pm for as little or as long as they'd like.  For therapeutic services offered on Monday and Thursday, children must attend at least 2 days per week to remain enrolled in the program.

    Services offered through Jacob's Mission Community Center are tutoring, guitar, piano, art, baking, cooking, basketball, pickleball, volleyball, tennis, football, dance, ukelele.

    Services offered through Living and Balance are Game Changers, Artistic Expression, Whisked Away, Lyric Lab, Cool, Calm & Collected, Gardening, Social Stars

Images, photos, and/or videos may be used to promote similar ASA Now events in the future, highlight the event and exhibit the capabilities of ASA Now. You release ASA Now, its officers and employees , and all persons involved from any liability connected with the taking, recording, digitalizing, or publication and use of interviews, photographs, computer images, video and/or sound recordings. By entering the event premises, you waive all rights you may have to any claims for payment or royalties in connection with any use, exhibition, streaming, web casting, televising, or other publication of these materials, regardless of the purpose or sponsoring of such use, exhibiting, broadcasting, web casting, or other publication irrespective of whether a fee for admission or sponsorship is charged.   • Program Attendance Requirement

    To ensure your child benefits fully from the Therapeutic Services offered by Living and Balance, there is a minimum attendance requirement of at least 2 full days per week. This prerequisite is necessary to maintain enrollment in the program. 
  • Transportation Acknowledgment

    Transportation coordination for this program is based on the rules and regulations of your insurance provider. Please note that Living and Balance Wellness Center does not provide transportation directly; we assist only with coordinating transportation through your insurance provider.
  • Medical Authorization and Consent to Treat: I hereby authorize ASA Now, Jacobs Mission Community Center, their agents, employees, personal representatives, affiliates, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, to consent on my behalf to any x-ray or other diagnostic examination, anesthetic, medical, dental, or surgical diagnosis or treatment, and hospital care, which is deemed advisable by, and is to be rendered under the general or specific supervision of any physician licensed under the laws of the jurisdiction where such diagnosis or treatment may be given, whether such diagnosis or treatment is rendered at the office of said physician, at a hospital, or at any other location, in the event my child requires urgent medical care. It is understood that this authorization is given in advance of any specific treatment or diagnosis, but is given to provide authority and power of treatment or hospital care which the aforementioned physician in the exercise of best judgment may deem advisable. I understand and agree, for myself, the child, the Parents, Foster Parents, and any other Caregiver or Guardian, that neither the School nor ASA Now, Jacobs Mission Community Center, their agents, employees, personal representatives, affiliates, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, shall be liable in any way in connection with any medical care provided to your child.

  • Accommodations for Children With Disabilities: Each child is an individual, and Jacob's Mission Community Center welcomes open and effective communication about a child’s particular needs as part of the enrollment process. Jacob's Mission Community Center is committed to complying with state and federal law regarding students with disabilities. It is very important for the child’s safety that Parents, Foster Parents, Caregivers and Guardians to fully inform the Jacob's Mission Community Center of any disability that may require an accommodation by the School during the enrollment process and before the child attends School. If your child has a disability that may require an accommodation by Jacob's Mission Community Center, please describe the disability and the proposed accommodation in the space below. Jacob's Mission Community Center will review this information and contact you to discuss whether reasonable modifications to the School’s policies and practices are available to accommodate your child’s needs. If the child requires an accommodation that the Jacob's Mission Community Center is unable and not required by law to provide, or if Jacob's Mission Community Center determines that the child’s presence poses a direct threat to the health or safety of any person, Jacob's Mission Community Center may rescind this offer of enrollment or request Parents disenroll the child and make alternative arrangements.
