REMINDERS:
- Neutering is a surgical procedure wherein the uterus and ovaries are removed from the female pet and the testicles are removed for the male pet to prevent them from unwanted reproduction.
- Your pet must be fasted 6-8 hours prior the surgery to prevent them from vomiting during the procedure.
- Pets must be placed in a cage to prevent them from escaping.
- Blood test is really needed for your pet to make sure that he/she is fit and healthy and without underlying illness.
- There is still possibility that after seven (7) days the male may still get females pregnant due to reserved sperm.

SURGERY CONSENT FORM and SPAY AND NEUTER WAIVER FORM AGREEMENT

MAKE SURE THAT YOU'LL BE ABLE TO READ AND UNDERSTAND THE FOLLOWING:
I, the owner of the pet named above hereby authorize BIYAYA ANIMAL CARE and to whoever Veterinarian they may designate to perform the spay/neuter surgery of my pet that is named in this form.

I understand that my pet will undergo anaesthesia and other medications needed to perform the surgery. In line with this, the operation presents some hazards, and that injury to and post-operative infection in, or death of, the animal may conceivably result. I understand and accept these risks to the animal. I withhold Biyaya Animal Care from any liabilities without proper medical examination and communication with our veterinary team.

BLOODTEST PRIOR SURGERY: I understand the animal will have to receive pre-operative bloodwork prior the surgery to assess if they are PHYSICALLY FIT. For pets aging two (2) years old below, CBC (Complete Blood Count) is required. For pets aging three (3) years old and above, CBC and Blood Chemistry is required.

I am allowing my pet to be neutered without vaccination, I am aware of the risks and complications involved. I understand the inherent risk of failing to maintain current vaccinations and waive all claims arising out of, or connected with, the performance of this procedure due to such failure.

FEMALE CATS AND DOGS: I understand that if my pet is PREGNANT, I consent Biyaya Animal Care to terminate the pregnancy.

FEMALE CATS AND DOGS: I understand that if my pet has PYOMETRA, (pus in the uterus), which will only be known during the surgery, I concent Biyaya Animal Care to continue with the procedure with an additional charge of Php 3000 (for cats) and Php 5000 (for dogs)

MALE CATS: I understand that if my pet has UNDESCENDED TESTICLES, I consent Biyaya Animal Care to continue with the procedure with an additional charge of Php 1000 (cat inguinal) / Php 2500 (dog inguinal) and Php 2000 (cat abdominal) / Php 3500 (dog abdominal).

Social Media Policy: No posting of surgical photos without the consent of organization, any misleading comments, claims, rumors and statement without going through our admin team communications are subject to legal action.
- I allow Biyaya Animal Care to spay/neuter my cat/dog.
- My pet was fasted for 6-8 hours (no food and water) prior the surgery.
- I understand that my pet will be under anesthesia and other medications that is needed after the surgery.
- I understand that the anesthesia will have reactions and my pet needs to rest for a few days to fully recover.
- My pet is in good health, has not been sick for the past 3 weeks and he/she is physically fit for the surgery.
- I will administer the prescribed medication after the surgery.
- My cat will have an ear tattoo/notch as a marking that it is already neutered.
- Spaying/neutering (KAPON) is a surgical procedure where in the uterus and ovaries is removed from the female pet and the testicles are removed for the male pet to prevent them from unwanted reproduction.
- I understand that a neutered pet will not be capable of reproduction; the male pets may still have the desire to mate due to their memory of the behavior, there is still a possibility that after seven (7) days the male may still get female pets pregnant due to their reserved sperm.
- If there's unusual bleeding, wound infection, lethargy, loss of appetite / drinking, or vomiting, I will immediately proceed to Biyaya Animal Care.
- I will put my pet in a secure carrier to ensure my pets safety and comfort.
  
  CARE AND GUIDELINES FOR PETS BEFORE AND AFTER SURGERY
- Your pet will undergo anesthesia, after the operation let it rest to recover.
- Pets should not be given food and water after the operation, allow 3-4 hours to pass before giving moderate food and water.
- Your pet may display inactivity for two (2) days after the operation.
- Pets should not be bathe for seven (7) days after the operation.
- Do not let your pet jump, run and play with other pets for five (5) days. Avoid letting them lick, bite or play with their wound.
- Be sure to give the prescribed medication to avoid any infection.
  Kindly place E-collar on your pet after the surgery

THIS IS FOR THE OWNERS WHO REFUSE TO UNDERGO THEIR PET/S BLOOD TEST AND INSISTED TO TO HAVE THEIR PET/S BE SPAYED/NEUTERED DESPITE OF HAVING ILLNESS, PLEASE SIGN THIS FORM.
PET'S NAME:
I, the owner has elected not to follow the recommendations of the Veterinarian and accepts responsibility for any consequences of that decision. The risks of not following the Veterinarian's recommendations have been fully explained to me. I fully agree that the Veterinarian shall not be held responsible or legally liable for the decision or any future consequences of my decision.
