Release Waiver for Creekside Canine, LLC.

By signing hereunder, I certify that I have been informed and understand that there is always some unavoidable risk of injury involved when working with or around live animals, especially animals with behavioral issues. I acknowledge that dogs can be inherently difficult to control and that not all dogs will be under control at all times resulting in the possibility of injury to myself, the dogs, my family members, or third parties.

I understand that I am providing a service around or direct care to live animals, which requires patience and attention to detail, and as such if I notice anything questionable about the health or welfare of the animal I will inform Katie Beth Dean, as soon as possible.

Therefore, I hereby release and agree to save and hold harmless, Katie Beth Dean, any affiliates or property owners from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I may sustain or which may be caused in any way by the dog(s) or any animals on the property. I specifically, without limitation, agree to fully indemnify Katie Beth Dean, Creekside Canine, LLC., and any affiliates or property owners for any and all such liability, claims, suits, actions, losses, injury or damage.

I understand that I may be photographed and/or recorded while at Creekside Canine, LLC. All such images will be the sole property of Creekside Canine, LLC and may be used to advertise, promote or publicize the facility.
