Proclamation/Certificate Request Form
The City of West Hollywood proudly issues Proclamations and Certificates of Recognition or Commendations to honor individuals, businesses, and organizations that have made meaningful contributions to the community.

Recognitions are strictly honorary and are issued as a courtesy to celebrate achievements and milestones that align with the City’s values. Please note that requests promoting political campaigns, events, or ideological beliefs contrary to the policies or positions of the City of West Hollywood will not be considered.

Certificates of Recognition or Commendation

These certificates celebrate:
- Significant personal or professional milestones.
- Individual contributions to society.
- Community events or achievements.
- Statements of congratulations.
Proclamations

Proclamations are ceremonial documents to:
- Commemorate community or organizational milestones.
- Celebrate events of historic or unique importance to West Hollywood.
- Recognize the work of individuals or organizations.
- Acknowledge extraordinary achievements or significant commemorative dates.
How to Request a Recognition

Timeline: Requests must be submitted at least two weeks before the desired completion date.

Community Connection: Recognitions are generally issued only for individuals, events, or organizations with a direct tie to the West Hollywood community.

Policy: The City reserves the right to modify, edit, or deny any request in accordance with its recognition policy.
