- The enrollment form will take approximately 10-15 minutes to complete. You will be able to save your progress and continue later, if needed.
- The information you enter and save in the form is recorded in real-time.
- For a better user experience, we encourage you to use a desktop to fill out this form.
- The following documents are REQUIRED to be uploaded as part of the submission:
  - Certificate of General Liability Insurance
    
    A declaration page is not an acceptable form of proof of insurance (Renew Financial does not have to be listed as a certificate holder)
    
    California – minimum policy requirement $1M
    
    Florida – minimum policy requirement $500k
  - Certificate of Workers Compensation Insurance
    
    California – If exempt, no documentation needed
    
    Florida – If exempt, the Workers Compensation Exemption Letter is required
  - Photo of Driver’s License for Principal Owner/RMO and Qualifier (if applicable). Please note that they will be required to sign our Participation Agreement

What do I need to upload?
The following documents below are REQUIRED to be uploaded in order to process your enrollment.
- Certificate of General Liability Insurance
