If you're considering canceling your membership, please note the following:
- Medical leave, military leave, or relocations over 25 miles from the gym may qualify for free cancellation.
- If you’ve completed your original 12-month commitment, your cancellation request will be placed on a 30-day pending status.
- Other cancellation requests may incur a $200 fee.
- To ensure a smooth process, all cancellation requests must be submitted through this form. Unfortunately, we cannot accept cancellations by phone, email, or in person at the gym.
