This Volunteer Release & Waiver of Liability releases The Beacon, a nonprofit organization organized and existing under the laws of the State of Texas and each of its directors, officers, employees, and agents (“Released Parties”).

Acknowledgment of Volunteer Status:
I, the Volunteer, understand that the scope of my position with The Beacon is that of a volunteer only and that I am not entitled to any compensation for my performance of duties as a volunteer. Further, I understand that I am not entitled to any employment benefits, of any kind or character, and am not covered by any Worker’s Compensation.

I acknowledge that I have received and read the Volunteer Policy, if applicable, for the program area I am volunteering with and will abide by its terms. I further acknowledge that The Beacon may modify or terminate my participation as a volunteer at any time for any reason, including situations in which I fail to abide by volunteer policies and procedures.

Release & Waiver of Liability
In consideration of being allowed to volunteer with The Beacon, I hereby release and agree not to hold Released Parties liable of any and all claims arising from my service as a volunteer.
Medical Treatment
In the event of a medical emergency, I (select one) * authorize The Beacon to contact my emergency contact below. I understand that The Beacon assumes no responsibility for any injury or damage which might arise out of or in connection with authorized emergency medical treatment.

I grant permission to The Beacon to photograph, record, or videotape me and grant The Beacon the absolute right, title, and interests in all media of me or my likeness in connection with my participation in volunteer services at The Beacon, without compensation to me.
