Authorization to Release Records

I authorize the carrier subscribers indicated on this application to do a complete background investigation in accordance with state and federal laws. I authorize my previous employers to release any information requested by these carrier subscribers and hold them harmless of all liability from the release of said information.

I hereby request and authorize to cause to be conducted, at any time, an investigation of my background for employment purposes, which may include, but is not limited to, any information relating to my character, general reputation, personal characteristics, criminal history, past work experience, educational background, alcohol or drug test results, or failure to submit to an alcohol or drug test, or any other information about me which may reflect upon my potential for employment gathered from any individual, organization, entity, agency, or other source which may have knowledge concerning any such items of information. I have completed this application of my own free will and hold harmless of all liability all companies, agents and associated parties for the use of this application.

Liability for Avoidable accidents:

If a driver causes an avoidable accident, they will be liable to pay the deductible cost as outlined in the company's insurance policy. Futher more, In the event of a avoidable acciddent, the company reserves the right to take appopriate disciplinary action, which may include suspension or termination of employment.

Safety and Performance Expectations:

Drivers are required to maintain a minimum Motive Safety Score of 85. If a driver's score falls below 70, it may result in termination due to safety concerns. Drivers maintaining a score above 85 will be recognized and rewarded for their performance. Additionally, drivers who receive a clean inspection at a scale will be eligible for special incentives or benefits as a token of appreciation for their compliance and professionalism.

Notice Period Requirement:

Driver are required to provide a minimum of 30 days' written notice before resigning from their position, failure to provide the required notice may result in withholding of the final paycheck to settle any outstanding dues owed by the driver to the company
