    This Service Agreement is for a participant in the National Disability Insurance Scheme (NDIS) and is made with Bridge to Friendships.
    This Service Agreement is for the below programs.

    Community Participation
    Recreational Activities
    Skill Developments Workshops
    Group Therapy Sessions
    Educational Excursions

  • Bridge to Friendships Service Agreement

    • MUST be completed prior to the commencement of any Programs/Workshops/Sessions.
    • The Service Agreement will be forwarded to the relevant Support Coordinator and Plan Manager.


    This Service Agreement is created to outline the provision of supports under the Participant’s National Disability Insurance Scheme (NDIS) plan. The Parties acknowledge that this agreement operates within the framework of the NDIS, a program designed to promote the life skills, social engagement, and economic participation of individuals with disabilities.

    Through this agreement, participants are empowered to exercise choice and control in achieving their goals and in the planning and delivery of the supports provided under the Bridge to Friendships program.

    Monday to Friday from 7th of April 2025 to 18th April 2025.
    9am until 3pm in Bridge to Friendships Centre

    Unit Price Per Hour $65.47

    Unit Price Per Session $392.82

    TOTAL PROGRAM COST  10 Sessions $3928.20

  • RESPONSIBILITIES OF BRIDGE TO FRIENDSHIPS (PROVIDER)

    Bridge to Friendships agrees to:

    • Deliver services as outlined in the program description.
    • Maintain accurate records of services provided to the Participant.
    • Communicate openly and honestly with the Participant.
    • Address Participant feedback actively.
    • Provide adequate notice if the service agreement needs to be terminated (refer to 'Ending this Service Agreement').
    • Safeguard the Participant’s privacy and confidential information.
    • Ensure all supports comply with relevant laws, including the NDIS Act 2013, associated rules, and Australian Consumer Law.

    Responsibilities of the Participant:
    The Participant agrees to:

    • Treat the Provider with courtesy and respect.
    • Communicate any concerns about the services provided.
    • Provide the required notice if the service agreement needs to be terminated (refer to 'Ending this Service Agreement').
    • Notify the Provider immediately if the Participant’s NDIS plan is replaced, suspended, or discontinued.

    Payments:
    Bridge to Friendships will invoice for services after they are delivered. If the Participant has nominated a Plan Manager, invoices will be sent directly to them. For self-managed participants, invoices will be sent directly to the Participant.

    Fees will align with the current NDIS Price Guide and are detailed in the Schedule of Supports. Payment for the full provision of services.

    Bridge to Friendships reserves the right to engage Debt Recovery Services for unpaid invoices, with associated fees passed onto the Participant per NDIA guidelines.

    Changes to This Service Agreement
    If changes to the supports are required, both parties agree to discuss and review the Service Agreement. Any changes must be documented in writing, signed, and dated by both parties.

    Ending the Service Agreement
    If the Participant wishes to terminate this Service Agreement or program, they must provide 1 weeks notice, and invoices will be issued accordingly.

    Participants can provide feedback, complaints, or suggestions for improvement by contacting Bridge to Friendships via:

    If the Participant is not satisfied with the response from Bridge to Friendships, they can contact the National Disability Insurance Agency (NDIA) by calling 1800 800 110, visiting an NDIA office, or visiting ndis.gov.au for further information.

    Agreement Signatures
    The Participant agrees to the terms set out above by Bridge to Friendships.
