2025 Arizona Tennis Classic 'Release of Liability' – MINOR

MUST BE SIGNED & SUBMITTED BY A PARENT OR LEGAL GUARDIAN

Each child requires a separate 'Release Form', and only one form is needed annually.

To participate in the 2025 Arizona Tennis Classic 'EVENT,' a separate form must be completed and signed by each minor's parent or legal guardian.

I, as the Parent or Legal Guardian of the Participant, acknowledge and voluntarily consent to my child, the Participant, participating in the 'EVENT,' the Arizona Tennis Classic, from March 2 to March 16, 2025. The 'EVENT' specifically includes Ball Crew and/or volunteer work, as well as other activities related to the Arizona Tennis Classic.

I understand that participation in the ‘EVENT’ is entirely voluntary, and I am not obligated to allow my child to participate. I acknowledge that no insurance coverage exists for any claims or damages arising from participation in the ‘EVENT’ offered by Arizona Tennis Classic.

As the Parent or Legal Guardian of the Participant, I agree to adhere to the customary rules and conditions for participation in the 'EVENT' hosted by the Arizona Tennis Classic at the Phoenix Country Club. I also agree to immediately notify an 'EVENT' representative if I observe any hazards during participation.

Acknowledgment of Risks

I am aware that participation in the ‘EVENT’ carries risks of injury, loss, or damage to the Participant, including but not limited to risks associated with running, walking, sitting, or standing during the ‘EVENT’; potential injuries from being struck by tennis balls or racquets; weather conditions; actions by spectators, players, or other volunteers; and both known and unknown actions arising from negligence, including by the ‘Released Parties’ (defined below). I knowingly and voluntarily assume all associated risks for all activities the Participant engages in.

Release of Liability

For myself, the Participant, and my heirs and legal representatives, I hereby release and hold harmless Arizona Tennis Charities, the Arizona Tennis Classic, the Phoenix Country Club, Phoenix Children's Hospital, the City of Phoenix, and all associated 'EVENT' organizers, sponsors, participants, affiliates, officers, agents, employees, volunteers, contractors, and staff (collectively, the 'Released Parties') from any claims, demands, actions, or causes of action arising from or related to participation in the 'EVENT,' including those resulting from acts of active or passive negligence.

Medical Consent

In case of injury or illness, I grant permission for the Arizona Tennis Classic 'EVENT' staff to provide medical treatment and secure emergency care assistance if necessary. I understand that the Participant's medical insurance is the responsibility of the Parent or Legal Guardian and will cover any medical expenses incurred as a result of participation. I assume full financial responsibility for any medical treatments or injuries sustained by the Participant during involvement in the 'EVENT.'

Health and Safety Acknowledgment

I understand that attending the 'EVENT' presents a risk of exposure to infectious diseases, including but not limited to COVID-19 and other health-related concerns. If the Participant shows symptoms of illness or encounters exposure to contagious diseases, I agree to immediately notify the 'EVENT' organizers and remove the Participant from the 'EVENT.' I certify that the Participant is in sound health and free of symptoms that prevent safe participation. Additionally, I agree to inform the 'EVENT' organizers in advance of any known food allergies or medical conditions that may require special attention to ensure the Participant's safety.

Image Consent

I grant the Arizona Tennis Classic, along with the other 'Released Parties,' the right to use photographs, videos, or other media captured during the ‘EVENT’ for marketing, promotional, or broadcast purposes. I waive any right to review or approve the media and acknowledge that no compensation will be provided for its use.

Indemnity Agreement

I agree to indemnify and defend the ‘Released Parties’ from any claims, damages, or losses arising from the Participant's involvement in the ‘EVENT.’

Legal Jurisdiction

I understand that the laws of the State of Arizona govern this agreement. The parties will resolve any disputes in an appropriate court located in Phoenix, Arizona.

Acknowledgment of Full Understanding

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. NO ONE HAS MADE ANY ASSURANCES OR GUARANTEES TO ME, AND I INTEND MY SIGNATURE TO SERVE AS A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
