Any member whose golf bag tag is chipped or in a bad condition may apply for a replacement. Request for the new golf bag tag can be made through the form below which must be accompanied by a recent photograph. For loss of bag tag, a fee of $16.35 (Inclusive of 9% GST) per tag shall apply.
