Informed Consent and Camp Agreement
1. The Camp’s non-refundable registration fee of $35.00 per week per child shall be paid at the time of initial application. A "Convenience Fee" will be applied of $2 per week when registering.
2. Families may select three days (Tuesday, Wednesday, Thursday), four days (Monday through Thursday) or five days (Monday through Friday).
3. Camp Tuition shall be paid in full by Friday, May 23, 2025. If tuition is not received by this date, the child(ren) will be disenrolled from the program and ineligible for a refund. Tuition total will be emailed directly to guardians and payable by check, cash or credit card via PayPal. If paid by credit card, a "Credit Card Convenience Fee" will be added.
4. Schedule changes can be made until Friday, May 23, 2025. Any changes made after this date may not be guaranteed and will not qualify for a tuition reimbursement or discount.
5. Additional siblings in the same family shall receive a 10% discount.
6. Tuition is non-refundable regardless of holidays, illness, vacation, inclement weather, or other “Acts of God”.
7. The Camp is closed on Friday, July 4, 2025.
8. The Camp will open at 8:00am and close at 5:00pm. A fee will be charged for any child not picked up before the Camp’s regular closing time. This charge shall be $1.00 per minute. Fees for late pick up are payable immediately. Consistent lateness will be cause for the child’s dismissal from the Camp.
9. The Camp reserves the right to deny, cancel, sever, or suspend a child’s enrollment at any time if the Camp, at the owner and director’s discretion, deem such action to be in the best interest of the child and/or the Camp.
10. Children may not attend the Camp ill. Children who become ill at camp must be picked up immediately. If the child will be absent, the absence should be reported to the Camp by 9am.
I hereby give my approval for my child’s participation in any and all activities prepared by Camp Magnolia. In exchange for the acceptance of said child’s candidacy by Camp Magnolia, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Camp Magnolia, and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.
In case of injury to said child, I hereby waive all claims against Camp Magnolia, including all counselors and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.
