CONSENT TO BACKGROUND CHECKS AND DRUG TESTING

1. Background Check: I authorize Ashley2 to conduct a background check, in its sole discretion, and compliance with applicable federal and North Carolina state laws. This may include verification of criminal records, employment history, and other relevant information. I understand that any offer for a booth rental may be contingent upon the results of this background check.

2. Drug Testing: I consent to undergo drug testing as part of the application process or during the term of the booth rental agreement, in Ashley2 ’s sole discretion, and as permitted by North Carolina law. I understand that any offer to rent a booth may be contingent upon passing this drug test. Refusal to submit to a drug test or a positive result may result in withdrawal of the booth rental opportunity or termination of the booth rental agreement.

CERTIFICATION AND SIGNATURE
I, the Applicant, certify that the answers given herein are true and correct to the best of my knowledge. I authorize Ashley2 representatives to verify any representations made by me either oral or written concerning personal employment, financial and/or other related matters as may be necessary in arriving at a booth rental decision. I understand that Ashley2 representatives may contact the individuals or organizations other provided as a reference in this process. I hereby release all employers, companies, corporations, credit bureaus, law enforcement agencies, schools, or persons from any and all liability in responding to inquiries in connection with this application. In the event of granting of a booth rental, I understand that false or misleading information given in this application (or any interviews) may result in the immediate termination of the booth rental agreement.
