These agreements serve to prepare participants for immersion into a sacred container of ceremonial initiation and retreat, establishing a framework which promotes a safe and streamlined experience for all involved. These agreements are assembled from many years of experience, and it is important that all members follow these rules without exception. Should they be disrespected, facilitators of the Crystal Heart Sanctuary will not hesitate to ask a participant to leave.

Facilitators of the Crystal Heart Sanctuary Church are invested in doing deep spiritual work, which can be very challenging at times. Occasionally, a participant can feel a strong urge to leave a retreat early. This can happen as energies are cleansed from the vessel, and unconscious patterns and limiting beliefs manifest resistance to this process. If a participant leaves early, the work is not only incomplete, but these energies of resistance can gain a stronger hold. For this reason, it is very important for each participant to cultivate a strong intention to complete the healing process they've committed to.

Crystal Heart Sanctuary Church must consider the safety and comfort of all attendees. It is our responsibility to make sure everyone is as safe and as comfortable as possible during their healing process. Participants will respect the facilitators and each other at all times. If a member is disruptive in any way, the facilitators maintain absolute discretion in determining when to ask a member to leave. Overt or disruptive negativity directed towards others will lead to expulsion from the retreat.

There is to be no sexual activity whatsoever between members and/or facilitators during a retreat. The environment demands abstinence from all sexual activity, including masturbation. Should one develop a sexual or romantic interest in another person while on retreat, it is essential that any physical expression of this is postponed until the retreat is over. All participants agree to honor these boundaries to respect and uphold the sanctity of the ceremonial container.

Crystal Heart Sanctuary facilitators are not medical practitioners. Ceremonies should not be seen, nor are they designed, as a substitute for psychiatric or medical care. It is each participant's responsibility to obtain a physician's approval prior to participating if necessary.

All alcohol and illicit drugs are strictly prohibited.

I, the undersigned, accept the conditions of participating in ayahuasca sessions held by the Crystal Heart Sanctuary and their team, and I declare that I am choosing to participate of my own free will. I have not been coerced into participating in sessions by the organizers or by any other person; the decision to participate is mine alone, and is based on my own personal assessment of the effects, the exclusion criteria, the potential risks and benefits, the focus of the session and the commitment of the people running it. I declare that I have answered all questions honestly during the screening process and I agree to provide all the relevant information about my medical history, my mental and physical health and any other information that may serve to protect my health during this process.

In general, ayahuasca is very safe. However, it can become unsafe if a person has contraindicated drugs or substances in his or her system, and could lead to a serious interaction, leading to injury or potentially even death.

Please take the time to read through our medical guidelines below. For your own safety and the safety of others it is imperative that you give your body sufficient time to purge street drugs and certain medications. We cannot stress how important this is to guarantee your safety and the safety of others. 

If you are currently taking prescribed medications that you are concerned will conflict with our sacred plant medicines, please consult the substances listed below and speak with your doctor before the date of your session. If you are unsure about anything that we have outlined, please contact us as soon as possible. 

Cannabis (marijuana) is a particularly important substance to avoid and thoroughly cleanse your system of in the weeks prior to your journey with us. While there isn’t a physically dangerous interaction, Cannabis has been known to act as a blockage for Ayahuasca and has been shown to prevent the sacred medicine from healing the body. Given that throughout the Ayahuasca ceremonies you will be in close relation to other participants who are particularly sensitive as they open themselves up to the healing ritual, a person intoxicated by Cannabis can darken the energy field of the medicine circle and prevent others from experiencing what Ayahuasca has intended for them. In the interest of others and yourself, please abstain from using Cannabis at least two weeks before and after retreat. 

Cocaine and Amphetamines, including MDMA (ecstasy) when used together with our sacred medicines can prove fatal. Stimulants such as those listed above contain Monoamine Oxidase inhibitors and when taken lead to an increased heart rate and raised blood pressure. Ingesting these alongside Ayahuasca can increase the risk of stroke, heart attack and cerebral hemorrhage. 

If we suspect that you have taken a prohibited drug or consumed alcohol upon or immediately prior to your arrival, we reserve the right to prevent you from participating in any Crystal Heart Sanctuary Church ceremonies. 

The alkaloids present in Ayahuasca interact with the serotonergic system in different ways: the beta- carbolines, for example harmine, are inhibitors of the MAO enzyme, which has its role in the degradation of serotonin and other neurotransmitters. Therefore, the use of Ayahuasca in combination with other serotonergic pharmaceuticals, such as antidepressants, can potentially cause adverse reactions such as ‘serotonin syndrome’, which theoretically can have serious consequences for one’s health. While the possibility of such adverse events occurring has not been confirmed, we advise that people that are receiving treatment with medication (including herbal preparations), or under the effect of any drug that acts on the serotonergic system, are extremely cautious when considering taking Ayahuasca. 

There are some physical conditions that may also prohibit participation. People with serious heart conditions and/or cardiovascular disease, high blood pressure, diabetes, a history of epilepsy and/or seizures, or any infectious or contagious disease should not participate.

Next to physical safety, it is important to take psychological safety into consideration. People with personality disorders or a history of serious mental disorders (such as borderline, bipolar, schizophrenia, etc.), as well as suicidal ideation and/or self harm are at risk if they use Ayahuasca. 

Contraindications: Drugs and Medications 

Any medication that contains MAO inhibitors (MAO-Is) are known to cause dangerous side effects when taken in conjunction with Ayahuasca and should be suspended. Any medication that has an effect on the serotonin system, including Selective Serotonin Reuptake Inhibitors (SSRIs), can induce serotonin syndrome, with potentially fatal results when combined with Ayahuasca. We advise that SSRI medications be suspended six weeks prior to a session to allow the system to clear. Some of these drugs may cause side effects when you stop taking them so you should allow plenty of time for these to subside and always consult your physician before changing your medical schedule. 

Please make sure you list any and all medications and supplements you are working with, and we will review everything you are taking before we confirm your registration.

These drugs and medications can be dangerous with Ayahuasca: 

▪ other MAOIs ▪ SSRI’s (any selective serotonin reuptake in- 7 hibitor) ▪ some antihypertensives (high blood pressure medicine) ▪ appetite suppressants (diet pills) ▪ medicine for asthma, bronchitis, or other breathing problems; antihistamines, medicines for colds, sinus problems, hay fever, or allergies (Actifed DM, Benadryl, Benylin, Chlor-Trimeton, Compoz, Bro- marest-DM or -DX, Dimetane-DX cough syrup, Dristan Cold & Flu, Phenergan with Dextromethorphan, Robitussin-DM, Vicks Formula 44-D, several Tylenol cold, cough, and flu preparations, and many others — any drug containing dextromethorphan/ DXM or with DM, DX or Tuss in its name.) ▪ CNS (central nervous system) depressants (xanax, ativan, etc) ▪ vasodilators ▪ antipsychotics ▪ barbiturates ▪ alcohol 

Please avoid the following herbs: 

• St. Johns Wort • Kava • Kratom • Ephedra • Ginseng • Yohimbe • Sinicuichi • Rhodiola Rosea • Kanna • Boswellia • Scotch Broom • Licorice Root • Cannabis • PHENIBUT

Using cocaine, amphetamines or MDMA (Ecstasy) with MAO-Is may cause a severe increase in blood pressure, increasing the chances for stroke and cerebral hemorrhage.

 • Using other serotonin agonists or precursors with an MAO-I can lead to serotonin syndrome. The main symptom of serotonin symptom may be a severe and long-lasting headache (the same symptom as MAO-I tryptamine interaction) and/or fever (as high as 40 °C / 104 °F or more) Other symptoms of serotonin syndrome may include rapid heartbeat, shivering, sweating, dilated pupils, intermittent tremor or twitching, overactive or over-responsive reflexes, hyperactive bowel sounds, high blood pressure. Severe serotonin syndrome may lead to shock, agitated delirium, muscular rigidity and high muscular tension. renal failure, seizures, and can be life-threatening.

I also attest that I will inform a Crystal Heart Sanctuary facilitator if I ingest any substance after submitting this registration form.

I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS Crystal Heart Sanctuary Church and its retreats, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from defective equipment or dangers on the property owned, maintained, or controlled by them, drinking of Ayahuasca, partaking of Rapé and/or Melipona, and/or participating in a Kambô Cleanse or because of their possible liability without fault. I certify that I am physically fit, have sufficiently prepared for participation in this ceremonial ritual, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems or use of any medications which preclude my participation in this activity. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the the Crystal Heart Sanctuary Church, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity. In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: Crystal Heart Sanctuary Church and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers;

(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.

I acknowledge that the Crystal Heart Sanctuary Church and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf. I acknowledge that this activity may involve a test of a person's physical and mental limits and carries with it the potential for death, serious injury, and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, drinking of Ayahuasca, partaking of Rapé and/or Melipona, participating in a Kambô Cleanse, lack of hydration, excessive hydration and actions of other people including, but not limited to, participants, volunteers, monitors, and/or producers of the activity. These risks are not only inherent to participants, but are also present for volunteers.

I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

1. CRYSTAL HEART SANCTUARY’s purpose is furthering the upliftment, enlightenment, spiritual realization, and general welfare of people throughout the world, the betterment of life on planet Earth, and other spiritual, humanitarian, and beneficent purposes. 

 
2. CRYSTAL HEART SANCTUARY  is dedicated to the revival and preservation of ancestral indigenous native Earth-based honoring cultures, lifestyles, sacred rituals, the healing sacrament of Ayahuasca, and loving humanitarian codes of ethics and conduct that protect Mother Earth and all of her living being from desecrations, pollutions, or abuses for economic gains. 

 
3. CRYSTAL HEART SANCTUARY is a creation of free men and women to associate with each other in the private domain according to their ministry and talents. 

 
4. CRYSTAL HEART SANCTUARY is a platform on which members may, for CRYSTAL HEART SANCTUARY’s good purpose, conduct all manner of private speech and business with CRYSTAL HEART SANCTUARY, members, and other associations – keeping all business in the private domain and utilizing the protections thereof. 

 
5. CRYSTAL HEART SANCTUARY is a private, unincorporated ministry that operates outside the jurisdiction of government entities, agencies, officers, agents, contractors, and other representatives, as protected by law. 

6. CRYSTAL HEART SANCTUARY lawfully stands on the authority our Creator gives to free men and women as revealed in the holy Scriptures, the Constitution of the United States of America, the constitutions of the several states of the union, and the Charter of Rights of Canada. 

 
7. CRYSTAL HEART SANCTUARY accepts Ayahuasca as a healing sacrament. This plant brew has benefitted humans for thousands of years and is central to our established religious beliefs and sacred ceremonies. 

  

8. Members claims the right to freedom of religion, free speech, petition, assembly, and the right to gather in association and access to our sacrament to assert our rights protected by those Constitutions, Charter, and Statutes. 

 
9. Members claim the right to be free from unreasonable search and seizure, the right to not incriminate ourselves, and the right to freely exercise all other unalienable rights as granted by our creator, our almighty God, and guaranteed by those Constitutions, Charter, and Statutes. 

 
10. Members decide for themselves which member(s) qualify to provide them counsel and advice concerning all matters, including but not limited to, physical, spiritual, healthcare, law, and any other matter, and may contract for counsel, advice, services, etc. they believe assist their ministry through CRYSTAL HEART SANCTUARY. 

 
11. Members have the freedom to choose and perform for ourselves the types of therapies and treatments we think best for diagnosing, treating, and preventing illness and disease and for achieving and maintaining optimum wellness, as well as the freedom to choose any types of assistance in lawful matters and any other private business activity. 

 
12. CRYSTAL HEART SANCTUARY will recognize as a member any person(s), natural or otherwise (irrespective of race, color, or religion) who has joined CRYSTAL HEART SANCTUARY or its social media groups and has agreed to the terms of membership, providing said person has not been sanctioned, exercised, or otherwise banned by CRYSTAL HEART SANCTUARY. 

 
13. CRYSTAL HEART SANCTUARY’s trustees, or their designee, may, at any time, terminate my membership should they conclude I am interacting with them or other members in a way that is contrary or detrimental to the focus, principles, and betterment of CRYSTAL HEART SANCTUARY. 

 
14. CRYSTAL HEART SANCTUARY is protected by the First and Fourteenth Amendments to the U.S. Constitution and outside the jurisdiction and authority of Federal and State Agencies and Authorities concerning complaints or grievances against CRYSTAL HEART SANCTUARY, members, or other staff persons. All rights of complaints or grievances will be settled by a CRYSTAL HEART SANCTUARY designee, committee, or tribunal and will be waived by the member for the benefit of CRYSTAL HEART SANCTUARY and its members. 

 
15. CRYSTAL HEART SANCTUARY and member activity is under common law rather than statutory law or regulatory law, which are creations of public government for the public. 

 
16. In my relations as member, I voluntarily change my capacity from that of a public person to that of a private member. 

 
17. My activities within CRYSTAL HEART SANCTUARY are matters of private contracts that I refuse to share with Local, State, or Federal investigative or enforcement agencies. I agree not to pursue legal action against a fellow member of CRYSTAL HEART SANCTUARY unless that member has exposed me to a clear and present danger of substantive evil and upon the recommendation and approval of CRYSTAL HEART SANCTUARY. 

 
18. I do not, and will not while a member, represent any Local, State or Federal agency whose purpose is to regulate and approve products or services or to conduct any mission of enforcement, entrapment, or investigation. 

 
19. I may, by written notice to CRYSTAL HEART SANCTUARY, terminate membership at any time. I will not misrepresent myself as being a member beyond the term of my membership. 

 
20. CRYSTAL HEART SANCTUARY may revoke my membership if I engage in abusive, violent, menacing, destructive or harassing behavior towards any other member of CRYSTAL HEART SANCTUARY. 

 
21. These pages consist of the entire agreement for membership in CRYSTAL HEART SANCTUARY. 

 
22. I enter into this agreement freely, without duress or coercion. 

 
23. I hereby exercise my right of “freedom of association” as guaranteed by the Universal Declaration of Human Rights (UDHR), the U.S. Constitution, and equivalent provisions of the various State Constitutions, as well as the Charter of Rights of Canada.
