1. If you are a new patient, an SMS will be sent to you within 24 hours of completing this form with a link to complete the Patient Intake Form.

2. The Patient Intake Form MUST be submitted before your consult. We need to complete the verification process before the practitioner calls you.

3. If you select a private billed or Medicare rebate consult on this form, an invoice will be sent to you via SMS including payment link. Payment MUST be received before your consult to avoid cancellation of your appointment.
