CPR Registration Form
Students are required to complete the information form provided below and submit a deposit of $15.00. The outstanding balance must be settled either in cash or through the online payment option available on this form on the day of the CPR session. Should a student have already paid the $15.00 deposit or the full course fee of $60.00, they will be exempt from the CPR session pricing chart below and will only need to sign an agreement with Med Care Pro LLC for the CPR session. Participants are anticipated to engage actively in practical CPR training while acquiring essential knowledge about CPR techniques. Upon successful completion of the course, students will receive their CPR certification card via email in PDF format from the American Heart Association. It is crucial to provide an accurate email address and complete information to ensure the certification is delivered without any issues.
