2024-2025 Community Assistance Grant Application

If awarded a grant, our organization will make every effort to send a representative to attend the FLSC Grants and Scholarships Awards Dinner on May 14, 2025. Checks are to be cashed or returned within 45 days of being awarded. Awarded organizations must provide a receipt, invoice, or other statement(s) detailing proof of funds utilization within 30 days of use. Any omission of information may cause the application to be delayed or denied.
