  • I understand and am aware that eyelash extensions are individual synthetic extensions glued to my natural lashes. This procedure requires me to keep my eyes close throughout the entire service. I agree to have eyelash extensions applied to my natural lashes or removed. By signing this agreement, I consent this service by Diana N. to be performed.

     
    I have informed my lash technician all my medical conditions. (Dry eyes, current allergies/sensitivities, current use of any eye products & etc.)

     
    This agreement will remain in effect for this service and future services & I understand this agreement is legal & binding. I have full read each statement and understand all information.

  • Have you had lash extensions before ?
  • Do you wear contact lenses or glasses ? (Please note if you wear contacts make sure to bring your contact case)
  • Do you have latex allergies ?
  • Are you pregnant ?
  • Can you sit/lay for 2+ hours ?
  • Have you had any eye surgery within the last 6 months ?
  • If you are experiencing pink eye or Covid/Flu/Cold symptoms or test postive for any before or on the day of your appointment, you will be responsible for canceling or rescheduling your appointment.
  • I understand and agree to follow after care steps. I understand that incorrect or lack of after care may results in damage to natural lashes or infection & I agree to take on that risk.
  • I hereby consent and authorize the person written below to take pictures and allow them to post me on social media platforms. (Instagram, Tiktok, or Facebook )
  • I hereby consent and authorize the person written below to perform the lash application.
