75th Annual National Youth Conference - Youth Sign Up Form

Please fill out the form carefully for registration. Child must be in Grade 5 through Grade 12. Youth Conference dates are July 11-17, 2026 at Ball State University in Indianapolis, IN. Parents/Guardians: Sign up regardless of your ability to pay the required registration fees. We encourage each participant to participate in the upcoming fundraising activities to help offset costs. - Elder David Bishop and Youth Minister Warren Blakney.
Youth Conference Fees are $325.00 for all participants.

Includes meals, lodging, and transportation to and from the conference.

$50 Deposit due by 5.15.2026

Final Payment due by 6.15.2026
