本人/吾等欲申請成為中國香港滑水總會有限公司(HKCWA)​會員，並承諾遵守水上安全規則及有關之法例，顧及一己及他人之安全。

I wish to apply for membership of the Hong Kong, China Waterski Association Limited (HKCWA) and agree to abide by the accepted code of safe waterskiing, wakesports and give due consideration for other water users.

本人謹代表本表格上所列之所有人等，同意作出以下有關個人責任之聲明。本人/吾等假若於香港滑水總會舉辦或安排之任何活動中不幸導致個人或他人身體損傷或死亡、或任何財物損失，或任何器材損毀、損壞、損失，一概不會向中國香港滑水總會，或與中國香港滑水總會合辦之機構，或中國香港滑水總會及有關合辦機構之執行委員、工作人員、職員及會員等作出任何形式之追究行動，謹此聲明。
In consideration of the Hong Kong, China Waterski Association Limited’s (HKCWA) acceptance of my application for membership of the HKCWA, in accordance with the details attached, I, for myself, my heirs, administrators and/or for the minor on whose behalf I am signing, hereby:
Severally waive all claims, expenses, rights, demands and actions of any nature for any personal injury to myself/ourselves or to a third party by myself/ourselves, or for any loss or damage sustained to my/our equipment or possessions arising from or in connection with my/our participation in waterskiing and wakeboarding organised and/or arranged by the HKCWA which I/we may have at any time against the HKCWA, their Officials, Servants and Agents, and the Association’s members.

本人/吾等明白滑水乃一種帶有危險性之運動，可能會導致受傷甚致死亡。
I understand the risks of personal injury and/or death inherent in participating in waterskiing and wakesports.​ 

作為中國香港滑水總會會員，我明白一旦本會清盤，本人需要分擔需要的款額予本會的資產，但不超過十港元(HK$10)。本公司為有限責任公司，在香港註冊成立的擔保有限公司
As a member of HKCWA, I understand I have a maxium liability of HK$10 in the event of liquidation of the company. The company is a limited liability company, limited by guarantee incorporated in Hong Kong

根據個人﹙私穩﹚條例，以上之個人資料將用作會員註冊之用，有關之資料將保留在中國香港滑水總會內，你有權隨時以書面方式，查詢或更改你的個人資料。
In accordance with Personal Data (Privacy) Ordinance, the above personal data will be used for registration purpose and will be maintained at HKCWA. You have the rights to request access to and correct your personal data.

申請人如未滿十八歲，必須由家長或監護人簽署，否則申請一概不會受理。
